Stonham
Stonham provide support services for vulnerable people into three main areas:
- Emotional support - providing emotional support aimed at building self confidence and raising self esteem.
- Practical support - Developing close links with community networks to allow better access to local resources such as personal development courses, other training, education, and employment, as well as the improvement of basic home skills.
- Advocacy and advice - providing benefits advice and access to resources which will improve skills and knowledge.
Support Plans:
Stonham will produce a personal support plan, tailored to meet the needs of the individual.
Initial realistic targets and goals are set and staff will work with them to achieve positive outcomes. The plans are reviewed each month to highlight areas where additional help may be required, and to measure progress made.
Support services provided by Stonham are:
- self contained flats for young people aged 16-20
- Floating support for 16-25 year olds
- Floating support for people of all ages
- Floating support for young parents
- Floating support for people with substance misuse support needs
- Floating support for people with learning difficulties

DISC
DISC specialise in working with those individuals and groups focusing on independent living and accommodation support for individuals who require support.
Through the national Supporting People Initiative they help hundreds of people settle into the community, especially those with a history of offending, drug and alcohol problems, mental health problems, domestic violence, young people leaving care, vulnerable young parents and homelessness.
The Independent Living Service Delivery Unit provides appropriate support to prevent homelessness and helps people to lead stable lives. In order to provide support to service users, help them to have more control and choice in their lives, we undertake a range of the following:
- Practical and emotional support
- Help and advice on tenancy issues
- Assistance with benefit claims and debt problems
- Assistance with healthcare issues
- Social activities to help strengthen social networks
- Access and links to community groups
- Assistance with home security issues and repairs
They have an 11 place floating support scheme for 16-25 year olds in the Easington area.

Mental Health Matters
Mental Health Matters provides a support service for people with mental health support needs and has 25 units across Easington. The service was established in 1994 and is jointly funded through Supporting People and Durham County Council Social Services Department.
Aims of the service
The service aims to provide outreach support to people with profound mental ill health and to enable them to live as independently as possible within their own homes in the community.
Support services
The service provided is diverse and tailor made to meet the needs of each service user. All service users are allocated a Support Worker who works with them towards an agreed support plan.
The service, which is staffed Monday to Friday, 9am to 5pm, offers both emotional and practical support.
Emotional support
- To encourage service users to express thoughts, emotions and fears.
- To assist service users to cope, especially during periods of distress.
Practical support
- To enable service users to improve their quality of life and achieve greater independence.
- To help regain or increase self-confidence.
- To enable service users to increase social opportunities and social interaction.
- To assist with daily living skills
- To develop budgeting skills
- The support staff also provide a low key monitoring of each service user’s mental health.
- The service does not provide personal care.

Kindstream Ltd
Kindstream provides support services to vulnerable adults to enable them to live independently in the community.
Cornwall Court- Comprises of 21 self contained flats in Murton which are fully furnished. There is a CCTV system on the site and all service users have access to a communal area. Individuals are issued with an assured shorthold tenancy and support services are funded by Supporting People.
There are staff on site 24 hours a day providing housing related support to service users to help them maintain their tenancy and build up confidence in order for them to move onto independent accommodation.
Fulwell Road- Comprises of 17 self contained one bed-room furnished flats in Peterlee. There is a CCTV system on site and a door entry system. Service users also have access to a communal area. All support services are funded by Adult and Community Services and tenants are issued with an assured shorthold tenancy agreement.
Kindstream outreach support services
Kindstream also offers outreach support services to vulnerable adults to help them maintain their tenancy. The support workers will work with services users to increase confidence to be able to manage independently and to access local amenities. Support workers will also work with service users to increase their confidence to be able to live independently in the community.

Home-Base
Seaham and District Training Trust have two services within the district; an accommodation based service for young females and a floating support service for young people in general.
Who is the Home-Base Project for and what’s our criteria?
The Home-Base Project is for young people aged 16-25 year olds. Floating support can be for males and females but the female accommodation is for females only.
Any person who falls under the referral criteria will be allowed to apply; this is in compliance with Seaham & District Training Trust Ltd.’s Equal Opportunities Policy.

Norcare
Norcare provides a support service to single men and woman from 18 years old upwards (no upper age limit) who have an offending background or who are at risk of offending to enable them to maintain or help them obtain a tenancy.
Norcare offer a flexible floating support service. Housing support workers are available between the hours of 8am and 8pm Monday to Friday and will meet clients in their own home at their convenience at least once per week.
The Service is free and is funded by the Supported People programme.
The service offers advice and practical support in:
- Help in obtaining benefits
- Applying for grants
- Obtaining furniture
- Support in achieving independent living
- Emotional support
- Sorting out debts and liaising between the probation service other statutory and voluntary agencies, especially the local authority and housing providers.
